  • 2019, 2020 and 2021 gross sales by quarter
  • Payroll data per employee quarter over quarter
  • Group healthcare costs by quarter per employee.
  • Copies of 941s for 2019,20 and 21
  • Completed PPP loan forgiveness application
  • List of any greater-than-50% owners
